B) 30 minutes post-ingestion
Damage is from generation of hydroxide radicals in the esophageal mucosa, causing caustic injury from high alkaline pH, rather than from electrical or thermal injury. Necrosis can begin as soon as 15 minutes after ingestion, with extension through the outer esophageal muscle layer within 30 minutes. Prompt removal is indicated emergently.
