Question: Otitis Media

About 70% of ear infections improve spontaneously. Which of the following patients is NOT a candidate for the “wait and watch” no-antibiotics option (for all patients assume well appearing otherwise)?

Which patient should not do wait & watch option for OM?

    E) 3 year old with unilateral OM, no fever, and cochlear implant placed 1 year ago
    Observation option is recommended as an option by the AAP guidelines for children aged 6-23 months with unilateral OM, fever 2 months prior, there are no anatomic abnormalities increasing risk of meningitis further, the child does not appear ill, and there is no spacer/positioner with the implant, oral antibiotics and close follow-up are acceptable; otherwise parenteral antibiotics should be used.
