More measles review: You are seeing a 15-month-old child who is visiting from Texas and presents with fever and rash for one day. The child has two older siblings and all are unvaccinated by parent choice.

Measles classically begins with a prodrome of fever, malaise, cough, coryza, and conjunctivitis. Koplik spots, an enanthem described in answer B, may be seen prior to the appearance of the rash. The rash classically begins on the face and spreads downward to the neck, torso, then extremities. Diarrhea is the most common complication, but pneumonia is the most common fatal complication in children. Rare serious neurologic complications include encephalitis (1:1000), ADEM (1:1000), and SSPE (subacute sclerosing panencephalitis 7-10 years after the initial infection, 1:10000-20000).
