Note: controversies are not meant to have a “right” answer – they are to see how most people are practicing. Would love your comments also regarding your thought processes and the evidence behind your decisions. We can learn from each other!

You are seeing a child who has been passing infrequent stools, and when the child does stool, the stools are hard.

What is the minimum age child for whom you would recommend Miralax?

The Miralax dosing information says “ask a doctor” for < 17 years old. However, many drug references including the PDR give off-label recommended dosing for infants (~0.5 to max 1 gm/kg/day). A retrospective cohort study of usage in the UK in 13,275 children < 2 years old identified use in 477 0-6mo olds and 3,378 6-12 mo olds. There were no confirmed electrolyte disturbance events. https://pubmed.ncbi.nlm.nih.gov/33587408/
