D) Tympanic membrane rupture from the blast wave energy
Primary blast injury is caused by the blast wave moving through the body. Examples include: blast ear (TM rupture and middle ear damage), blast lung (lung parenchyma injury such as contusion), blast brain (TBI eg contusion), blast eye (globe rupture), blast belly (abdominal hemorrhage and perforation). Secondary blast injury is caused by debris displaced by the blast (either from the device itself or nearby items) – answer C. Tertiary blast injury is caused by the person being displaced by the blast energy and impacting something – answer B. Quaternary blast injury involves all other types of injury: burns, toxins, smoke inhalation, radiation, and psychological – answer A. The sprained ankle doesn’t really fit into any of the categories of blast injury.
