D) Cow’s milk protein sensitivity
The protein sensitivity causes an allergic proctocolitis, and may present with hematochezia or melena. It most commonly presents in the first 3 months of life. Patients may have anemia and eosinophilia. Treatment is by changing to a hydrolyzed or amino-acid based formula (infants are often sensitive to soy formulas also). C. diff is often falsely positive in young infants, and testing is generally not recommended in < 3 year olds.
