C) With the flange at the lips and the tip at the angle of the jaw
Oral pharyngeal airways are sized by finding the airway that when held at the side of the face with the flange at the level of the lips, the tip reaches just to the angle of the jaw. Nasal airways are sized by finding the airway that most closely approximates the distance from the nostril to the tragus of the ear. If the nasal airway has an adjustable flange that can be slid up and down to change the effective length, then choose based on the airway with a diameter that best matches the nostril size and adjust the length to be from the nostril to the tragus of the ear.
