You are preparing to defibrillate a 2-year-old child with ventricular fibrillation who weighs 13 kg using a manual biphasic defibrillator that has increments of 2, 3, 5, 7, 10, 20, 30, 50, 70, 100, 150, and 200 Joules.

The first defibrillation attempt dose is 2 J/kg and should be followed immediately by 2 minutes of high-quality CPR. 2 J/kg for this patient is 26 J, which can be rounded to the nearest available amount, 30 J. The second defibrillation attempt if the first is unsuccessful is 4 J/kg. The dose four this patient is 52 J, and can be rounded down to the nearest available amount, 50 J. Subsequent attempts can be performed with doses of 4-10 J/kg with a maximum at the adult dose of 200 J. Of course, it may be easiest to use a Broselow tape or similar length-based resuscitation tape, which will give the appropriate doses.
