Question: Triage

You are triaging patients from a multi-casualty incident. The following patients are seen:

Patient A: Approximately 2 years old, ambulatory, crying, large 8cm parietal hematoma, obvious right humeral deformity

Patient B: Approximately 5 years old, unresponsive, apneic after airway positioning and 5 rescue breaths, weak palpable pulse

Patient C: Approximately 3 years old, responsive to painful stimuli, breathing at 40 breaths per minute, very weak palpable pulse

Patient D: Approximately 7 years old, unresponsive, breathing at 30 breaths per minute, weak palpable pulse

Which patient receives first priority?

    Using the Jump Start algorithm http://www.jumpstarttriage.com/The_JumpSTART_algorithm.php patients can be divided into minor (green), delayed response (yellow), immediate response (red), and deceased (black). Priority should be given to those in the immediate group; using the limited resources in a multi-casualty incident on patients in the deceased group will not maximize the number of survivors. In a NON multi-casualty scenario, full resuscitation efforts would be attempted in those in the deceased group, but priorities change in multi-casualty incidents with limited resources.
